Director of Case Management

Job Description Job Description: Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and coworkers in accordance with our Service Excellence Standards and Core Values. Translates the McLeod Health vision and goals into practical strategies. Responsible for operational management of the Case Management Department. Responsible for implementing processes that support and promote efficient patient throughput across the healthcare continuum resulting in optimal clinical outcomes, appropriate utilization of resources, and accurate reimbursement. Provides clinical management and leadership to staff in nursing roles with a goal of advancing the practice of Hospital Case Management. Utilizes and interprets data to identify factors allowing for rapid change/course adjustment to stay on target and achieve operational and program goals. Develops and implements standard work including regulatory requirements and operational policies and procedures for Clinical Case Management services. Understands and is able to implement rapid cycles of change which results in successful short?term and large?scale organizational change. Responsible for assessing clinical competency of staff and reinforcing skills supporting efficient patient throughput along the continuum of care. Complies with all applicable government regulations and contractual obligations relating to provision of Clinical Case Management services. Assists in the planning and development of an annual budget; analyzing variances; initiating corrective actions. Develops and leads a high performance team by participating in recruiting, selecting, orienting, and training of Case Management staff. Motivates team members to success through a collaborative leadership style. Completes annual performance evaluations according to policy. Coordinates and oversees multi-disciplinary team care meetings to assure appropriate throughput. Responsible for assessing, reporting, and communicating program status on a periodic basis to all program stakeholders. Develops and uses the LEAN system to continuously evaluate and address internal and external opportunities for improvement. Contributes to team effort by accomplishing related results as needed. Schedule: Full Time; 80 hours biweekly. Experience: Minimum 3 years of hospital management experience required; Minimum 3 years of case management experience required; Minimum 3 years of compliance related experience preferred. Education: Bachelor of Science/Arts or equivalent work experience; Current license as Registered Nurse required; Certification in Case Management or Utilization Review preferred. Physical Requirements: Carrying objects (30 lbs. or less) from one location to another; Sitting in normal seated position and/or standing/walking long distances for at least 8 hours per day; Reaching to answer telephone, manipulate mouse, and complete data processing. Work duties require occasional strenuous effort. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
